Q: How to enroll as a new patient?

A:  If you meet all the requirements listed under New Patients you can fill out and submit the New Patient Forms using the Dentrix Patient Portal. For a full breakdown of the new patient requirements and detailed instructions on how to access the patient portal click here.

Q: Where is our office located?

A: 128 North Blakeley Street, Monroe, WA, 98272. Our office is located in downtown Monroe behind Thrive Fitness and next to the Post Office. To locate us on google maps visit our Contact page.

Q: What are our office hours?

A: We are open Monday to Thursday from 8:30 am to 5:00 pm and on Friday’s from 7:00 am to 1:00 pm. Holidays may affect these hours.  For more contact information click here.

Q: What is required to set up your billing/insurance?

A:  The requirements include but are not limited to current x-rays, health history, detailed personal info, completed financial agreements, signed privacy act, and verification of your insurance policy. Q: What services do we provide?

Q: Do we accept after hours patients with dental emergencies?

A: Yes!  Our office message after hours will give you instructions for reaching Dr. Crandall directly so he can determine the nature of the dental emergency and provide appropriate options.

Q: Are there additional fees for missed or cancelled appointments?

A: There can be fees assessed for missed or late cancelled appointments.  We do request 48 hours notice to give us time to offer those appointments to other patients in need and keep the schedule filled for our staff and doctor.
